Medicines at School

If your child requires medication during school hours a parental agreement form must be completed to give staff your permission to administer the medication.  The medication must be handed personally to either Bev Smith, Carol Pracownik or a staff member at Breakfast Club.

If your child also requires medication while at Otters Den (the extended schools provision) an additional parental agreement form must be completed for extended school staff to administer medication.
 

No medication will be administered unless it is prescribed and the medication is in the original box.
